4.A – For Students Not Applying for ESC Subsidy

Submit these documents:

• PSA Birth Certificate (Photocopy)
  
• Grade 6 Report Card or Completion Certificate
  

4.B – For Students Applying for the ₱13,000 ESC Subsidy

To be considered for ESC, submit all required documents on or before September 30.

​❗Note:  Submission does not guarantee subsidy approval. The school evaluates ESC applications based on financial need and vulnerability, and that applicants under the HOME+ and SCHOOL Program ESC applicants may be given priority. Failure to complete requirements by the deadline will void the application.

Required ESC Documents:

1. ESC Application Form

     Submission Options:
   • Onsite: Print and fill out by hand, attach a 2x2 photo, and submit at school
   • Online: Fill out using a PDF editor and upload (we will attach your photo)
   • Delivery: Submit in a long brown envelope via courier
2. PSA Birth Certificate (Photocopy)
3. Grade 6 Report Card or Completion Certificate / ALS / PEPT Certificate
4. Two (2) 2x2 ID Photos (White background)
5. Proof of Income (choose one):
   • Certificate of Employment
   • Certificate of Indigency (from Barangay)
   • Latest Income Tax Return (ITR)
   📌 Ensure details in the proof of income match the ESC Application Form.

1. In-Person Submission at SMIS
   • Visit the school at:
     St. Mark Integrated School, 3 Gloria 2 Subdivision, Tandang Sora Ave., Quezon City
2. Courier Delivery (e.g., Grab/Lalamove)
   • Address to:

     Include your child’s name and grade level in the courier note/chat.
3. Online Submission via Google Form
   • Upload scanned documents via: [Google Form Submission Link]
   • Make sure all files are clear and readable.
